Team Members
What is the Team Members Section?
The Team Members section identifies people or roles who need to be part of the improvement activity. All those affected by the process are stakeholders and should be identified as a team member.
The Team Members may be identified as ad hoc, meaning they will not attend every meeting, but will be called on when their expertise is needed.
For larger, more complex projects, help may be needed to identify team members and stakeholders. A tool that is useful in doing this is the SIPOC diagram.

A SIPOC Diagram can be used to identify stakeholders and members of the project team. All Suppliers and Customers identified in the SIPOC are potential team members.

How to Identify Team Members
Use a SIPOC Diagram to help identify team members.
Step 1: Complete a Process Flow Diagram
Complete a high-level process flow diagram:
- This should include at least 4 and no more than 8 process steps
- Each process step name should be succinct - use one verb + one noun
Step 2: Identify Outputs
What outputs does the process create?
- These are tangible things (e.g. reports, diagnosis) and should be fairly high-level
- Up to 3 or 4 main outputs should be included
Step 3: Identify the Customers
Who receives the outputs?
- These 'customers' may be internal (e.g. other staff), or external (e.g. patients, whānau)
- Each output should have a customer
Step 4: Identify the Inputs
What triggers the process? What does the process need?
- These are tangible things (e.g. reports)
Step 5: Identify the Suppliers
Who are the people or departments that provide the inputs?
- Sometimes the Suppliers are the same as the Customers
